Re-Order Form

To choose your style of mug & glaze color, refer Catalog, Terms and Conditions, and Glaze Chart Please also make note of the following:

• We require a minimum of 24 mugs
• Payment terms are 100% down for new orders. Upon receiving a tracking number for your order, your credit card will be charged a second time for shipping.  We accept Visa, MasterCard and American Express.
• Overruns/ Underruns: Due to the firing process, we produce a quantity that is more than what is ordered. Every effort is made to ship the exact quantity ordered. Orders within a variance of +/- 10% are complete and will be shipped and invoiced.
• Please allow 5-7 weeks for your delivery once order is placed.  

Once your order is entered, a Sales Order confirmation and a Payment Receipt (if applicable) will be sent.

Please provide Sales Tax Exempt form if located in the state of Wisconsin (only if applicable).
